Abstract Objective To investigate the clinical elfects of modified electric convulsive therapy ( MECT) combinedwith paliperidone in the bipolar allective disorder (BPD) patients with manic episodes. Methods 99 patients with BPD manic episodes admitted from May 2021 to May 2023 were collected and divided into drug group (n=49) and MECT combined group ( n = 50 ) according to different treatment regiments. The clinical eficacy, control of bipolaidisorder and mania, inflammatory factors, neurotransmitters and adverse reactions were compared between the twogroups. Results After treatment , the total elfective rate in MECT combined group was higher than that in drug group, showing a statistically significant difference ( P<0.05 ). After treatment, the clinical global impression-BP (CGI-BP)score , Bech-rafaelsen mania rating scale( BRMS ) score, serum levels of tumor necrosis factor (TNf ) -α . interleukin( IL) -1, IL-1β and adrenocorticotrophie hormone ( ACTH) in MEC'T combined group were lower than those in druggroup. The serum levels of brain derived neurotrophic factor ( BDNF) and glial derived neurotrophic growth factor( GDNF) in MECT combined group were higher than those in drug group ( P<0.05 ). The ineidenee of adversereactions had no significant difference between the drug group and MECT combined group ( P>0.05 ). Conclusion MECT combined with paliperidone has good clinical elficacy in the treatment of BPD patients with manic episodes,which improves neurotransmitter, reduces inflammatory factors, helps to control manic state and improves thecondition of bipolar disorder.
